The platform aims to assist health systems and medical groups in launching scalable CCM service lines, empowering healthcare providers to proactively care for their patients between visits. It focuses on improving patient wellness and generating sustainable monthly reimbursement. Phamily caters to medical groups and healthcare organizations of various sizes, including primary care, internal medicine, family medicine, family practice, and geriatrics.
