Formstack is used by our whole organization. We use Formstack to handle all of our feedback forms, surveys, and inter-departmental requests. We've tried other survey and form services that could tackle part of our needs but not all of them. Formstack is ideal since it can easily be embedded into our website, integrates with other services we use and has robust email features.
Pros
Conditional Logic: we haven't found another service that does this quite as well as Formstack.
Simplicity/User-Friendly: Incredibly easy to build even the most robust of forms.
Embedding.
Integrations: Integrates with tons of other services/platforms that keeps our ecosystem running smoothly.
Cons
Sometimes the form builder is slightly clunky or buggy.
Complicated conditional-logic gets hard to follow/troubleshoot.
Seems like there is unnecessary feature or design switches that come out of nowhere.

Formstack is very well suited for surveys and feedback forms. We haven't really found a limitation or a place that it isn't appropriate.

We use forms from Formstack to implement applications for pilot testing, as well as for certain lead retrieval tasks that Hubspot is unsuitable for. We also use it as a tool internally to manage group events and to collect information about the team, as well as a type of internal ticketing system for various requests (design, copyediting, etc.)
Pros
Compatibility with old browsers.
Fairly easy to build forms, even for users who haven't been explicitly trained.
Versatility in form building styles, which allows us to use it for many different purposes.
Integrations with other software platforms, like Hubspot.
Cons
Managing form designs in Formstack is very difficult, and we've had numerous problems with design/styling.
Certain features are missing from the form builder, like "same as above" functionality.
The form storage backend is difficult to navigate, and it is difficult to find forms in any way but searching for them.
Sometimes features in the form builder require reverting to the old form builder to implement.

For well-funded institutions with a large number of forms needs, Formstack is well-suited for a variety of tasks. However, the styling limitations held us back numerous times and the we've reached the form limit frequently and have had to upgrade our account. For smaller, less well-funded organizations, the cost would quickly get prohibitive if they use it like we do.
Formstack is used for multiple departments in our business. We use it to get feedback from guests that visit the waterpark and campground. This allows us to make modifications to create a better experience for our guests. We use it for hiring as well! From the start to finish Formstack allows us to gather the info we need from our family members. Everything from taking reservations to guest follow-up is or has been handled by Formstack!
Formstack has been a very useful tool that had provided us the ability to connect with our fans by capturing their information, providing easy to use sign ups for events, and making efficient user-friendly purchases. Other features such as usage of the data and seamless integration on our website have made the experience even better.

Formstack is not an application specifically designed for tickets sales, so its lacks features directly related to the sports industry.
Mobile app has limited functionality.
The ability to reset your password for encrypted forms - if you forget it or one of the only people who knew it leaves the organization you need to lose all the submissions. This might be a legal thing however, which is understandable.

[Formstack is well suited for:] Sign-ups, Surveys, Sales, Web sales. I'm not sure about situations where it wouldn't be suited.
We use Formstack across two departments for both our annual season subscription pre-sale orders as well as more regularly as a tool to manage our volunteer sign-ups for upcoming performances, lost & found reporting, and volunteer inquiries. Formstack has allowed us to easily integrate some of these items on to pages of our website such as the lost and found reporting and volunteer inquiries. The volunteer sign-ups are now a much more efficient system given our needs and make it both easy for our volunteers to fill out, especially given that many of them are older and not as technically savvy, and quick for our volunteer coordinator to download and assign performances. Overall, it is a relatively inexpensive system but can help with several functions.
Pros
Formstack has a very user friendly interface for setting up the forms/surveys. A lot of the items are easy to drop in and customize for basic users. The terminology is very straight forward.
For more intermediate to advanced users, there is a lot more functionality that can open up and the ability to incorporate responsive forms is great. The shorter a form is or appears to the person filling it out is beneficial, and Formstack allows you to show only applicable fields rather than having fields that are present that someone has to skip over.
The integration with 3rd party systems is also pretty easy to manage. I also utilize Formstack for a non-profit I volunteer for and there, we have Formstack synced to Google Docs for some data hosting and Constant Contact for our email host and had no trouble connecting the systems and even customizing some filtered email lists.
Cons
As I stated in one of my pros, Formstack integrates well with Google Docs for data hosting. The reason why we utilize this is because there is not an easy table view of data in Formstack. You can view each complete entry separately or you can view charts and averages, but it's not easy to reference otherwise.
There isn't an easy way to undo when creating/editing forms. There have been a few occasions where I have accidentally deleted a portion of credit card information and have had to delete all of the rest and reinsert and connect the elements.
Managing the themes can sometimes be tough to do as there isn't a clear 'preview' option for that screen and you have to toggle back and refresh a form to see how it will layout on the page and go back and forth a bit to determine the best look/fit.
Likelihood to Recommend
If you are looking to get more functionality without needing a massive amount of customization or web design/programming skill then Formstack is an excellent option. There is a lot of integration built in to the system and it is laid out in a very straight forward and easy format. You will have some restrictions on the layout of the forms, colors, and backgrounds but the look of the forms are not unpleasant or difficult to use. Overall it is a very good product for the associated costs.
